This is actually wrong I feel because it is not providing appropriate records or information about the process of LA or ICA and they should be records of this and why people withdraw. I feel that each couple should have information recorded in their own words why a withdrawal or non-assessment occurs.
I know of more and more ICA cases where the couple have been approved and then with re approval occurring every two years at the two year mark they are 'advised' (very strongly) to take a 'deferment' - i.e. pushed out of the process.
Kay I would advise to communication mostly by email, so that you have a record of information. I would even advise after an important phone call, you follow this up with an email and even request for your email system to record a "have read" email from the person at the other end.

HI everyone,
Thanks for all the advise, part of me is wondering if she is waitong for me to bite (so to speak).
If she hasnt called by Tuesday to re-schedule, I will contact her and I have decided in my next interview to be a little more to the point and we will see how we go. If I am not happy at the end of the week I think we will speak to the director about our concerns.
